Individual or Joint Account


Use this application to open a regular (taxable) investment account for an individual investor or a joint account. 

Individual Retirement Account (IRA)


Use this application to open an Individual Retirement Account (IRA). Types of IRAs that can be opened include: Traditional, Roth and SEP. If transferring a pre-existing IRA from another company to Nicholas Funds, please also complete an IRA Transfer Form and include a copy of your most recent statement. If rolling over a pre-existing retirement plan, such as a 401(k), 403(b) or 457, simply complete an IRA Application and we will assign an account number which you can provide to your current plan administrator. Corporations, Partnerships and Corporate Pension Plans


Use this application to establish a mutual fund account for a corporation or other legal entity. Be sure to include a copy of the articles of incorporation or other documentation of legal organization.

Uniform Gift to Minor


Use this application to establish an investment account for a minor. An adult must be designated custodian on behalf of the minor until the minor reaches the age of majority. This may vary from state to state. 

Coverdell Education Savings Account Application


Use this application to establish an education savings account for the benefit of a minor. Investment gains in this account grow tax-deferred and withdrawals are tax-free if used to pay for qualified educational expenses at an accredited elementary school, secondary school or college. If transferring a pre-existing Coverdell Education Savings Account from another company to Nicholas Funds please also complete an Education Savings Account Transfer Form and include a copy of your most recent statement.
